Children ages 0-12
All Information will be required::
Parent/Guardian Photo ID
Proof of Current Address (utility bill,
medical bills, lease, etc)
Proof of Household Income (Food stamps and/or recent pay stubs)
Birth Certificates for all children 12 and under (shot/medical records with birth date on it. We need to see the birthdate.
Clothing and Shoe Sizes for Children
Deadline is 12/09/22. Failure to complete the entire application and upload required documents will result in denial of your application. You will receive an
