Planning for Your Life Advance Directives
This is an opportunity to discuss and complete your Advance Health Care Directive with expert guidance and support. This event is sponsored by the Humboldt Advance Care Planning Coalition whose vision is to see that all adults in Humboldt County have the information, opportunity, and encouragement they need to complete an Advance Health Care Directive so that their wishes may be expressed and respected. A Notary Public will be available to assist with finalizing documents if needed. Please bring a current photo ID.
